This is because you have short filenames and long filenames:

There's really no need anymore for short filenames. You can disable it:

I've also been trying very hard to create "colliding" filenames so we can test this more, but it's proving impossible. For example, if you create a file called "An Example.txt" and then try to create a file called "ANEXAM~1.TXT" in the same folder you can't (Windows won't allow it), so I'm not sure how you have "realaudio.htm" and "REALAU~1.HTM" in the same folder. I'm guessing the files were created using a 16-bit program? What file-system format is the source?
