Do you mean it is deleting without prompting, or that it doesn't prompt (thus, doesn't do anything in respect of that file)? If you mean the latter, that's normal if you run the profile Unattended.
For example, using When > Changes is Unattended by default; if you do not have the non-default option 'Run interactively, i.e prompt me if required' on that settings page enabled, it won't prompt you (it won't delete anything either, but whatever the conflict is, it will remain unresolved). This applies whenever/wherever you have it set to Prompt in one place, but then run it so that Prompts are disabled...so in your case, suggest set the 'Run interactively' option on When > Changes page
Note: if it encounters a 'promptable' situation, but you are running it in Unattended mode, it will by default record the fact it could not prompt you (and thus, took no action) as a Warning in the log. The 'class' of event (Warning, Error, no log entry at all, etc) can be adjusted in the Modify > Log settings page of the profile.
