No, it doesn't (as per KB I gave you a link to). Files or folders that are filtered out (and/or de-selected) are not deleted, they are Ignored.
Note that you didn't specify what you selected when (after adding the *\*.doc filter) the program asked if you wanted to de-select \document.doc but I tried it with both settings (Include and Exclude), and in neither case was \document.doc deleted from Destination (nor did I expect it to be, nor should it be, as per KB)
Hence I cannot replicate what you describe, and what I describe is how it works.
BTW, stating your profile doesn't have rules is not the case - all profiles have rules (under Decisions-Files).
